Increased insurance deductibles ($2,000.00 - $5,000.00), increased co-pays ($50.00 - $100.00), decreased healthcare benefits has forced many healthcare providers including Dr. Williams Chiropractic Office to shift the responsibility of receiving payment in full from the patient's insurance to receiving payment from both patient and the patient's healthcare insurance company. To assist the patient in receiving reimbursement from their healthcare insurance company, Dr. Williams Chiropractic Office provides a computer generated itemized statement which includes the diagnosis codes, service codes and fees required for payment and reimbursement. Initial Visit patients are required to pay a deductible of $100.00 for partial payment of the cost(s) of consultation, orthopedic and neurological examination, physical therapy as well as mis-alignment correction of the spine and/or extremities. Follow-Up Visits by patients are required to pay a deductible of $50.00 for re-evaluation, physical therapy and mis-alignment correction of the spine and/or extremities. Nutritional supplements (Herbs, Minerals & Vitamins ) are not covered by the patient's healthcare company and therefore must be paid for by the patient. Supports and/or braces are not covered by the patient's healthcare insurance company and therefore must be paid for by the patient.
