Upon completion of the Initial Paperwork, you may e-mail or fax the completed paperwork to Dr. Williams Chiropractic Office (please call Dr. Williams Chiropractic Office before faxing paperwork)!
 
A New Patient is greeted with a smile and a warm handshake!

A New Patient is escorted to their seat and asked to fill out the needed office and health questionaire sheets!

A New Patient is asked to please notify any member of the chiropractic office team (including Dr. Williams) when they have completed their paperwork! 
 
The chiropractic office team member will review all paperwork to make sure it has been filled out correctly, dated and signed.

The New Patient will be asked to follow the chiropractic office team member to a consultation room to meet and consult with Dr. Williams!

Dr. Williams explains to the New Patient the procedures that must be performed before Dr. Williams Chiropractic Office can accept them as a chiropractic patient!

The New Patient consults with Dr. Williams regarding why they are seeking chiropractic care!

A X-ray examination of the neck, lower back and/or the area of complaint(s) will be performed!

A orthopedic/neurological examination will be performed!

Physical therapy will be performed!

Dr. Williams will review x-ray examination while New Patient is receiving physical therapy!

If necessary, New Patient will receive emergency spinal adjustment(s) to relieve muscle spasms and/or nerve pain/pressure!

If necessary, New Patient will receive orthopedic support(s) to help relieve muscle spasms and/or nerve pain/pressure!

The New Patient will be scheduled As Soon As Possible to review consultation, x-rays, and orthopedic/neurological examination findings!

The New Patient will pay for all services rendered by Dr. Williams Chiropractic Office
